2. Article 16 of the Regulations of the People's Republic of China on the Administration of Drug Administration;
3. Article 14 and Article 17 of the Measures for the Administration of Drug Operation Licenses.
Application Condition 1. The ongoing investigation has not yet been settled or the administrative penalty has not been carried out yet;
2. If provided the false materials or acted fraud in the application process, there will be no acceptance of application for at least of one year period in accordance with the "Administrative Licensing Law";
3. Other laws and regulations stipulated do not accept conditions.
4. Matters need attention
Where a pharmaceutical retail enterprise changes the licensing matter of a "drug business license", it shall be submitted by the enterprise, and the license matter shall not be changed without approval.
To change the registration matters, should apply for registration of change to the City Council, within 30 days after the acquisition of the changes of registration approval ofthe industrial and commercial administrative departments. To change the name of the enterprise, it shall be changed according to the "Enterprise Name Change Notice"issued by the industrial and commercial administrative departments. The "Enterprise Name Pre-approval notice" cannot be used as the basis for business name changes.
Application Materials (1) to change the legal representative, the person in charge or the quality control person in charge
1. drug business license change record application form;
2. The original and a photocopy of the education certificate, the qualification certificate and the title certificate; and the personal resume and the professional personnel qualification certificate, the letter of appointment of the legal representative of the enterprise, the person in charge of the enterprise, the person in charge of the quality control (the original file will be returned after check);
3. A copy of the Enterprise
4. Online entry information for the provincial administrative approval system (provide according to the provincial bureau website administrative approval system client program requirements).
(2)to change the business address (including increase or decrease of the operating area)

The enterprise shall provide the certificate of the property right or the right of use of the premises or warehouse place (provide the Certificate of Registration of the Housing Lease if located within the city area, provide the Certificate of Registration of the Housing Lease or other proof of business use if located within the Shangjie District and the counties); If the business, warehouse place is to set in the residential type of house, a written statement or description of the situation agreed to change the residential house to business use from the house owner shall be provided;
3. Business location map, flat layout, and the surrounding environment description;
4. A copy of the Enterprise
